I, hereby certify that my animal(s) is/are in good health and have not been ill with any contagious condition. I have read and understand the following:

1. I understand that I am solely responsible for any harm caused by my animal(s) while they are in the care of Doggy Walking or it’s staff or associates.

2. I further understand and agree that in putting my animal(s) in the care of Doggy Walking, they have relied on my representation that my animal(s) is/are in good health and has/have not harmed or shown aggressive or threatening behaviour towards any person or any other animal (unless specified). Doggy Walking may at any time remove an animal from its care if he/she exhibits behaviour which could be harmful to other animals or people.

3. I further understand and agree that Doggy Walking and its staff/associates will exercise due diligence and reasonable care while caring for my pet, and will not be liable for any problems that may arise. I hereby release Doggy Walking, it’s employees and contractors of any liability of any kind whatsoever arising from my animal(s) being in their care.

4. I further understand and agree that animals can sometimes receive minor cuts and scratches when outdoors or playing. If an animal becomes otherwise ill or injured and is in need of immediate care, Doggy Walking will transport the animal to the closest veterinary hospital and attempt to reach the owner while the animal is in transit. If the owner is unreachable, Doggy Walking will have the veterinarian proceed with any treatment deemed necessary. Owners will assume all financial responsibility for veterinary treatment.

5. I agree for Doggy Walking to use photos or video recordings of my animals for marketing or any other purpose.

6. You agree that your dog does not damage household items or gardens and if they do while being minded, you will cover any replacement costs including labour if required.

You agree that your dog does not usually consistently bark (especially at night). Barking can be caused by anxiety from being in a different home environment than they are used to. If barking persists to the point where it can not be tolerated, you agree Doggy Walking and the minder can take the following steps:
• Place the dog into a boarding kennel environment (any remaining minding days will go towards cost of boarding)
• Seek vet advice for anxiety medication
• Place an anti-bark collar on the dog
Any costs associated with the above options will be reimbursed by you, the owner

8. Declined payments will attract a flat fee of $10 per transaction. Late fees of $10 per day will apply if payment into our bank account is overdue. Cancellations with less than 2 weeks notice (1 month for school holiday bookings) will be charged the fee as though the service has been delivered.
